
Inline adoc files when they are included exactly once. Also fix language tags because this inlining gives us better information on what language the code is written in.

All RPG IV references I could find define the "name" operand as required (e.g. \http://publib.boulder.ibm.com/iseries/v5r1/ic2924/books/c0925083696.htm#HDRZZREAD).

Maybe this rule applied to RPG III, but we don't support RPG III.
